Based on the NN/g Skill Mapping Template, I made a lot of improvements after putting that to practical use.

Reorganized UX skill sets into four main categories (Research, Interaction Design, Content Strategy, and Visual Design).
Included Leadership and Core Qualities to assess important skills outside of UX that are indicators for success.
Each category is broken down further into more specific areas with definitions for each item to improve the team’s shared understanding of each metric.
Created two versions: 1) All-in-one view for fully transparent teams or manager only assessment and 2) Linked spreadsheets for private individual scorecards that populate for the manager.
Tabbed layout for individual scorecards and improved current and future state visualizations.
